Rimington Model 11, Please Help

Hi everyone, this is my first post and it feels good to be in the company of other people who have something in common.

Anyway, at times my old Remington Model 11 wont feed the shell into the chamber. I'm posting a photo of the main issue that it has. The shotgun is setup correctly with the ring on the correct side of the spring for heavy loads.

It's easy to un jam, all I have to do is push that plate down below the base of the shell and it loads. But before doing so I have to hold the bolt handle back or the bolt will slam my finger into the chamber. Can anyone tell me what the source of this malfunction is?
